Skilled nursing care provides teachings about disease process and management.

Skilled nursing care such as:

Home Health Aides are caring individuals who help patients to carry out their activities of daily living.

These professionals will have the following duties:

  • Bathing and dressing

  • Transfer/ambulation

           

A healthcare social worker is someone who helps people access services and education related to health problems.

Their goal is to have the patient achieve and maintain a state of optimal health so they can function in their daily lives.

The social worker acts as a patient advocate, educator, advisor, coordinator of care, and strives to preserve the healthcare and dignity of the population they serve.

Physical therapy services are delivered by licensed physical therapists and physical therapy assistants.

These services may include:

  • Regaining strength
  • Managing pain
  • Enhancing safety
  • Gait training

  • Home exercise program
  • Prosthetic training
  • Use of assistive device

       

Speech therapists help patients to relearn speech skills that may have been lost due to brain injury.

These professionals also aid patients in re-learning how to chew and swallow safely after a stroke or other brain injuries.

A speech therapist will work to improve communication skills and provide training for the patient to learn compensatory skills such as sign language and lip reading. This is particularly suited for individuals who have hearing and speech impediments.

These services may include:

  • Speech/swallow evaluation
  • Language disorder treatment
  • Speech articulation therapy
  • Dysphagia treatment

       

Occupational therapy practitioners enable people of all ages to live life to its fullest by helping them promote health, and prevent—or live better with—injury, illness, or disability. 

Common occupational therapy interventions include helping people recovering from injury to regain skills, and providing support for older adults experiencing physical and cognitive changes. Occupational therapy services typically include:

  • an individualized evaluation, during which the patient/family and occupational therapist determine the person’s goals.
  • customized intervention to improve the person’s ability to perform daily activities and reach the goals, and
  • outcome evaluations to ensure that the goals are being met and/or make changes to the intervention plan.
